A battery back-up siren has its own internal power source, so it keeps sounding even if the vehicle battery is disconnected or the siren wiring is cut. Disconnecting the battery is one of the standard ways of silencing a basic alarm, which is why back-up sirens are worth the difference on a vehicle left unattended.
The 24 volt siren is for trucks and commercial vehicles running a 24 volt system. Fitting a 12 volt siren to a 24 volt vehicle will destroy it.
